This unit is about giving support services within the limits of an individualized plan. Individualized plans have different names in different organizations.
How you will be assessed
You will be assessed through:
- Assessor observation on the job
- Supervisor reference (if the assessor is not the supervisor)
- Review of your workplace documents (e.g. procedures, case notes, individualized plans)
- Interview/assignment based on the requirements below.
Practical

You will be assessed in the workplace with suitable facilities, equipment and resources, including individualized plans, equipment outlined in the plan, and infection control policies and procedures.
You need to show that you have used individualized plans for supporting three individuals.
Show your assessor your workplace and the forms and procedures for doing assessment. Explain how it works as you go. Your assessor may ask you any questions on how it works. Most organizations have clear procedures.

- What are your organization’s documentation and reporting requirements?
- Describe the roles and responsibilities of different people:
- carers and family
- person being supported
- health professionals
- individual workers
- supervisors.
- Describe the communication between them. How does it help or cause difficulties when giving support?
- Explain two different service delivery models in your industry sector.
- What are the legal and ethical requirements for the folowing:
- privacy, confidentiality and disclosure
- duty of care
- dignity of risk
- human rights
- discrimination
- mandatory reporting
- role boundaries – responsibilities and limitations?
- How are these legal and ethical requirements applied in an organization? In individual practice?
- What factors affect people requiring support?
- What practices help workers to maintain and develop their skills?
- What are the signs (indicators) of unmet needs? How should you respond to each one?
- How should you assess risk?
- What are the common risks in providing individualized support?
- How should you respond to each common risk?
Note: Your assessor may also ask you a variety of
what ifquestions.
Detailed requirements
1. Determine the person’s support needs El 1/PC, RS L
- Clarify your role and ask for help with aspects outside your knowledge, skills or job role.
- Confirm the details of the plan with them, and also with family and carers when appropriate.
m- Make sure they understand their rights and how to lodge a complaint.
- Work with them to identify actions that support the plan, and that promote their independence and rights to make informed decisions.
- Prepare to give support according to their plan, preferences and your organization’s policies, protocols and procedures.
2. Provide support services El 2/PC, RS. O,In,L
- Interact with the person in a way that develops and maintains trust.
- Respect and include the family and/or carer as part of the support team.
- Respect individual differences to give the person maximum dignity and privacy.
- If you need equipment, assemble it as and when required according to procedure and the plan.
- Give support according to:
- the plan,
- the person’s preferences and strengths,
- your organization's policies, protocols and procedures,
- duty of care, and
- dignity of risk requirements.
- Ask for help when you cannot give appropriate support.
3. Monitor support activities El 3/PC, RS O,In G
- Monitor your work to maintain the required standard of support.
- Involve the person in discussions about how support services are meeting their needs and if there is any need for change.
- Identify aspects of the plan that might need review and discuss them with your supervisor.
- In your discussions with the person and your supervisor, do so in a way that supports the person’s self determination.
4. Complete reporting and documentation El 3/PC, RS O,In,W
- Do any informal and formal reporting required by your organization. This includes reporting your observations to your supervisor and completing written reports and storing them.
- Identify and report signs of additional or unmet needs of the person and refer them.
- Identify and respond to risks within your role and report them to your supervisor as required.
- Keep all information about the person confidentiality and private.
- Follow your organization’s policy and protocols in all the above.
5. Other.
- Manage tasks and contingencies on the job.
